A 3 mm mirror gets in anywhere and shows you a keyhole. When the access is not the constraint, the extra two millimetres are worth having – Ø 5 mm nearly triples the reflecting area, which at working magnification is the difference between finding a feature and scanning for it. Is 5 mm too big for microsurgery?
It depends entirely on the access you have made, which is your call and not ours. Hahnenkratt list 3 mm, 5 mm and the oval as equal members of the range rather than a progression – they are different shapes for different windows.

Why black rather than polished steel?
At high magnification with coaxial light, a polished stem beside the mirror face puts a hotspot in the eyepiece and flattens the contrast on everything else. The DLC finish leaves only the optical face reflecting.

What about the oval?
The 3 x 6 mm oval gives you 6 mm of length through a 3 mm gap – the shape that suits a long, narrow field better than a circle does.
