One of Kohler’s two apicectomy retractors. Both carry the same 40 x 10 mm serrated blade; the only difference is the bend. This one turns the blade square to the shaft at 90 degrees and is the anterior reach of the pair.

90 or 45 degrees?

Access, not function. The 90 degree blade sits square to a flat labial plate, which is the anterior approach. The 45 degree version lays the blade over for a shallower posterior reach. Trays that do both usually carry one of each.

Why serrated

A plain blade holds the flap by pressure alone and can creep. The toothed edge is there to keep the blade seated once it is placed.
