Endo-Top is Cerkamed's irrigation needle: 30G, 25 mm, closed rounded tip with a side vent, so the irrigant leaves sideways at low pressure instead of straight down the canal. Cerkamed's wording: "The product acts by on irrigation using a turbulent flow of fluid, which is dispensed through the side hole of the needle causing low pressure on the tissues." Luer lock, ethylene-oxide sterilised, one hundred to the box.

› Cerkamed's three steps: "Open the blister. With a Luer-lock thread, mount the needle onto a syringe filled with the chosen fluid. Remove the needle sheath and check the needle for visible defects or fractures." Then expel a little fluid to confirm the flow before it goes in the canal.
› From the IFU, the depth the web page leaves out: "measure its length and then set the stop (endostop) on the needle to a depth of 1-1.5 mm less than the measured working length. Inserting the needle to this depth minimises the risk of it getting stuck in the canal."
› Cerkamed's IFU on the delivery: "The liquid should be introduced into the root canal with continuous pumping movements." And: "do not use too strong a liquid jet, as this may cause the liquid to penetrate the periapical tissues. If resistance is encountered during needle insertion, the needle should be carefully withdrawn."

Because an open-ended needle pushes the irrigant apically and a side vent pushes it at the wall. Cerkamed describe the effect as turbulent flow at low pressure on the tissues. That is the design argument; how far short of the apex to set the stop is the clinician's call and the irrigant manufacturer's directions.
A 0.3 mm cannula is what Cerkamed make it, and 25 mm is the working length of most syringes it will sit on. For a canal that has only been scouted with a C-PILOT ISO 010, 25 mm it may not reach far; once shaped, it does – to 1–1.5 mm short of working length, which is where Cerkamed's IFU has the stop set. Do not pre-bend it: the IFU says to avoid twisting and bending.
