Finding the MB2 without opening the tooth any further

A second mesiobuccal canal you can see on the CBCT and not with a file? That is an 008 job, and in an upper molar it is a 21 mm job. This is the size most calcified canals genuinely start at: fine enough to enter a constricted orifice, stiff enough to tell you something when it stops. VDW credit the file's high bending strength to a special thermal hardening process, and the tip is deliberately inactive so the canal leads it rather than the other way round.

› Look first, probe second – an 008 is not a substitute for magnification and a good chamber floor clean.
› Watch-wind, do not push – small arcs and light pressure; force at 008 buys a ledge.
› Keep the flutes clear – irrigate and recapitulate rather than asking a lubricant to do it. Where a chelating gel is part of your protocol, File-Eze™ is the 19% EDTA option.
› Retire a stressed file – six per blister exists for exactly this reason. Single use, per VDW and the UK position since April 2007 (CMO(2007)5).

008 or 006 for a calcified molar?
Start at 008. Keep 006 for the canal where even this stops – it is finer, and correspondingly more fragile.

What comes next?
Once 008 is loose to length, move to 010. When that is loose, you have a glide path and the motor can take over.
