The widest apex in the range, in the shortest length

Short roots, limited opening? Twenty-one millimetres suits a second molar, a patient who cannot open far, or a root that does not need the reach – and it buys handpiece clearance where a 25 mm file fights the opposing arch. This is the Large – the widest canals in the range – in that length.

The widest apex, on the lowest taper

A 0.45 mm tip is a substantial apical preparation, and the Large carries it on a .05 variable taper – the lowest in the range. That combination is deliberate: the file that opens the apex furthest is also the one that takes the least coronal dentine per millimetre.

It is the file for a wide canal, an immature or resorbed apex, or a retreatment where previous instrumentation has already spent some of the coronal thickness and there is none to give.

Where a conventional large file would cost you the crown

Take a fixed-taper instrument to a 0.45 mm apical size and the coronal third goes with it. The regressive geometry is what lets this file finish the apex while leaving the pericervical dentine that carries the restoration.

As with every size above the Primary, gauging decides. The Large is a step-up, reached because the Medium sat loose at length – not a starting point.

On the taper figure: .05 is the nominal designation at the tip. The taper eases coronally rather than holding .05 up the shaft, and Dentsply publish no average-taper figure for WaveOne® Gold, so none is quoted here.

Questions we get asked

› Do I still need a glide path? Yes. Secure the canal to length with a #10 hand file and confirm patency, then the Gold Glider creates the glide path in the same reciprocating motion. The instructions for use require a secured, patent canal first.

› Is the Large a starting file? No. The sequence starts at the Primary; the Large is where gauging takes you when the Medium is loose at length.

› What motor do I need? Any motor carrying the WaveOne setting – X-Smart® Pro+, X-Smart® Go, VDW.SILVER® and VDW.GOLD® RECIPROC®.
