Real-T-Endo™ · #12 upper left first premolar · radiopaque

The tooth that teaches an ovoid access – before someone perforates one

Why do upper premolars get perforated? Because a round bur meets a chamber that runs buccal to palatal, and the cavity keeps getting wider in the wrong direction. The Real-T Endo #12 is built for exactly that lesson: two distinct roots, one buccal and one palatal, with the pulp chamber lined centrally between the buccal and lingual cusps. Acadental calls it terrific for practising an ovoid access preparation to learn how to prevent perforation of the tooth – and because it radiographs fully, a near miss can be seen rather than argued about.

Two distinct rootsOne buccal, one palatal – two independent working lengths in a manageable tooth
Central chamberLined centrally between the buccal and lingual cusps, which is what makes the access ovoid

Radiographs fullyAcadental: the entire tooth can be radiographed

› Access is the exercise – the Real-T arrives unopened, so cutting the outline is the skill being assessed.
› Thin roots, real risk – the danger zone in an upper premolar is the furcal wall, and a film will show how close the bur came.
› The see-through version – Endo 3DP #12 is pre-accessed and radiolucent, for shaping practice rather than access.

Is this a beginner or an intermediate tooth?
Intermediate. It is the natural second exercise after #08: two canals, real perforation risk, still only one film to interpret.

What does a good access look like here?
Ovoid, following the chamber buccopalatally, with both orifices visible and no unnecessary extension mesially or distally. That outline is the assessment.

Which files suit small premolar canals?
A hand-file glide path first, then a conservative single-file system – the RECIPROC Minima range is designed around narrow preparations.
