A bigger apical preparation that doesn't cost you the crown

Necrotic case wanting a larger apical size? A canal where 0.25 mm simply isn't enough? The old trade-off was that going up in apical diameter meant going up in taper too, and paying for it in coronal dentine. R35 breaks that: a 0.35 mm tip on a 6% apical taper that reduces to just 2.6% averaged along the blade – the lowest-taper way in the range to reach that apical size.

The taper goes down as the tip goes up

Read the range in order and the design intent is obvious. R20 is 20/.08v, R25 is 25/.08v, and R35 is ISO 35/.06v – the tip diameter climbs while the apical taper steps down from 8% to 6%, and the average along the blade falls from 3.6% to 2.6%. So the larger apical preparation is genuinely apical: you are not dragging a wider body up through the coronal third to get there. For anyone working to minimally invasive endodontic principles, that is the whole point – apical size is a clinical decision, pericervical dentine is a structural one, and this range lets you make them separately.

R35 and R45 may be used directly, without employing the intermediate instruments, owing to their reduced overall taper.

Where it fits

› After R20 or R25 – when the case calls for a larger apical diameter than the smaller files leave.
› Directly, in suitable anatomy – where the canal is not narrow and you want the apical size in one instrument.
› Necrotic cases where a larger apical preparation is part of your disinfection strategy, without the coronal enlargement that usually comes with it.
› Still narrow? Start with R20 as the instructions direct, then step up.

Questions we get asked

Can I go straight to R35?
In wider anatomy the instructions leave the starting size to you, and the reduced overall taper is what makes going directly practical. In narrow and challenging canals they are specific: begin with R20.

Isn't a 0.35 mm apical size aggressive?
Apically, it is larger by design – that is why you would choose it. What it avoids is the coronal cost: at 2.6% average taper the body of this file is slimmer than most 4% and 6% instruments of a smaller tip size.

Do the published study figures apply to R35?
No, and we will not pretend otherwise. The International Endodontic Journal laboratory study tested R20 and R25 against RECIPROC® blue R25 in deliberately narrow canals. R35 was not in it. Its case rests on the geometry, not on those percentages.

Do I still need a glide path?
The instructions require one if the file stops progressing, plus a patency check with an ISO 010 or 008 hand instrument. Do not force any file down a canal.

Can I reuse them?
No – sterile blister, single use.
