A medium canal at the end of a long root

Long roots are not automatically narrow ones. A canine or a long-rooted premolar that takes an ISO 20 hand file passively to working length – directly there with a gentle watch-winding movement, no filing action – but refuses an ISO 30 is a medium canal, and the file for it is R40. In 31 mm, with five radiopaque depth markings to read length by.

› It finishes at ISO 46. VDW put an ISO 46 at 1 mm from the apex with this instrument, so gauging with a hand file one size larger tells you when to stop.
› Bigger tip, lower taper. 0.40 mm at the tip but 6% over the first 3 mm rather than the R25's 8% – apical size without the coronal enlargement that usually comes with it, which matters more the longer the root.
› Or the step up from R25. Where a hand instrument one size larger than the 31 mm R25 reaches full working length, this is the next file.

Which hand file reaches at this length?

Is one instrument enough for a long canal?
The instrument prepares the whole canal, and the single-use rule is per molar rather than per case. In an extremely curved canal VDW advise one file per canal, which is worth budgeting for on a long, curved root.
