Gauged, not guessed – the medium canal file

The most useful thing about this system is that the size is decided before the motor starts. A canal visible on the radiograph, an ISO 20 hand file that goes passively to working length – directly there with a gentle watch-winding movement and no filing action – and an ISO 30 that does not: that is a medium canal, and R40 is the instrument. In the everyday 25 mm length.

› Apical size without the coronal cost. A 0.40 mm tip, but 6% over the first 3 mm rather than the R25's 8%, regressing above it – the taper falls as the tip diameter rises, which is what keeps coronal dentine where it is.
› It finishes at ISO 46. VDW put an ISO 46 at 1 mm from the apex with this instrument. Gauge with a hand file one size larger: light tug-back about 1 mm short and no further means you are done.
› Or step up from R25. Where a hand instrument one size larger than the R25 reaches full working length, the apical third wants enlarging and this is the next file.

Do I have to work through R25 first?
No. The sizes are chosen from the canal rather than stepped through in sequence, which is the difference between a single-file system and a rotary one. Where the selection test says R40, start there.

And retreatment?
R25 does the removal; R40 or R50 follow it if the apical third needs enlarging afterwards. Zuolo and colleagues (International Endodontic Journal, 2013) found the system left the least residual gutta percha and sealer of the retreatment approaches compared, at 4.57%.
