That is the whole test, and VDW want it done before the motor starts. A canal visible on the radiograph that takes an ISO 20 hand file passively to working length – directly there with a gentle watch-winding movement, no filing action – but refuses an ISO 30 is a medium canal, and R40 is the instrument for it. In 21 mm, on an 11 mm shaft, for short roots and awkward access.

› A bigger tip on a lower taper. 0.40 mm at the tip but 6% rather than 8% over the first 3 mm – apical size without the coronal enlargement that usually arrives with it.
› It finishes at ISO 46. VDW put an ISO 46 at 1 mm from the apex with this instrument, so gauging with a hand file one size larger tells you the preparation is complete.
› Or step up from R25. If a hand instrument one size larger than the R25 reaches full working length, the apical third wants enlarging, and this is the next file.
Can I go straight to R40?
Yes, where the selection test says so – the sizes are chosen from the canal, not stepped through in sequence. That is the difference between a single-file system and a rotary one.
What if I need more than ISO 50 apically?
Use R50, and above that a hand file. VDW's own note: hand files larger than ISO 50 with a 2% taper go to length very easily in a canal that already carries a wider taper.
