The largest TruNatomy® file, on the short shaft

A maxillary central incisor, some palatal or distal molar canals. Those are Dentsply Sirona’s own examples of where the Large belongs. It is the newest and largest TruNatomy® shaping file, 46.02v, and it comes after Medium – never instead of Prime. This is the 21 mm version, for short teeth or where opening is limited.

Sizes as Dentsply Sirona print them. The “v” means variable taper; the taper shown is the average along the file, not the taper at the tip. Motor settings from the Solution Guide.

What this file does

›  Where Dentsply Sirona say to use it. Step 13: “Use TruNatomy® LARGE in larger and straighter canals only, such as maxillary central incisors, some palatal or distal canals of molars.”
›  After Medium. Prime loose at length, then Medium, “and subsequently with LARGE if needed”.
›  Newest size in the range. Not in the 2019 launch literature; the current brochure lists it as now available.

Where it sits in Dentsply Sirona’s sequence

1  Prime, then Medium. Prime loose at length with clean apical flutes, Medium to full working length (step 13).

2  Large, if the canal is larger and straighter. Guided gently to full working length (step 13).

3  Confirm and fit. Confirm working length by hand with the Large, then try in the Large cone (step 14).

Questions we get asked

Can I use the Large in a curved molar canal?
Dentsply Sirona limit it to larger and straighter canals and give their examples. A curved canal belongs to Prime, Small or Medium.

Why is the taper lower than the Medium?
The .02v taper sits on a much larger ISO 46 tip. Dentsply Sirona publish the size, not the reasoning, so we will not invent one.

Which points and cone?
The Large ones, matched to the last instrument used.

Which motor settings?
Dentsply Sirona give one setting for every TruNatomy® file: 500 rpm and 1.5 Ncm. Set it on any of the three endo motors we stock – X-Smart® Go · X-Smart® Pro+ · VDW.SILVER® RECIPROC®.

When do I throw a file away?
Dentsply Sirona’s procedure (step 11): inspect the cutting flutes routinely on removal for unwinding and straightening, and if deformation is noted, discard it and use a new TruNatomy® instrument.
