When 25 mm leaves you short of length

Canines, long-rooted premolars, and the occasional molar that measures longer than it looked. The 31 mm R25 is the same instrument as the everyday one – a 0.25 mm tip on an 8% taper over the first 3 mm, M-Wire®, S-shaped core, non-cutting tip – with the reach a long root needs and an extra pair of depth markings to read it by.

› Five markings instead of four. 18, 19, 20, 22 and 24 mm, radiopaque, so a long root still gives you a reference on the film.
› Same instrument, same protocol. The size is chosen from the radiograph and a passive hand file, not from the length – and in most cases it is R25.
› Match the rest of the case to the length. A 31 mm canal needs a hand file that reaches it too. Where the range runs short, we say so below rather than sell you something that stops at 25 mm.

Which hand file for patency at this length?
C-PILOT® is made in 19, 21 and 25 mm only, so it will not reach here. Use VDW K-Files ISO 010 in 31 mm – the same manufacturer, the size the directions ask for, in a length that reaches working length. ISO 008 and 015 are there too, the latter for a glide path if the instrument stalls.

Do I still avoid an initial glide path?
In the majority of canals, yes – De-Deus and colleagues (International Endodontic Journal, 2013) took the R25 to full working length without one in more than 93% of canals. Patency is still confirmed with an ISO 010 hand file between passes, and if the instrument stops advancing you withdraw it and create a glide path rather than forcing it.
