Small 20.04v – not needed in most cases, and exactly what you want when Prime stalls

Dentsply Sirona are candid about the Small: “not needed in most cases”. Every TruNatomy® case starts shaping with Prime. The Small is the file you move to when Prime does not progress easily, or for severe curvatures and narrow canals – and then you finish with Prime. This is the 25 mm version, the middle of the three lengths.

Sizes as Dentsply Sirona print them. The “v” means variable taper; the taper shown is the average along the file, not the taper at the tip. Motor settings from the Solution Guide.

What this file does

›  When, in their words. The Solution Guide: “Small not needed in most cases but can help for severe curvatures or narrow canals” and “If the Prime does not progress easily use the Small then finish with Prime”.
›  Slim wire, off-centred section. Dentsply Sirona build the shaping files from dedicated heat-treated NiTi on a 0.8 mm wire – against up to 1.2 mm for most generic variable tapered files – with an off-centred cross-section and regressive taper.
›  Always followed by Prime. Step 12 takes the Small to working length, then Prime to working length “to optimize the shape”.

Where it sits in Dentsply Sirona’s sequence

1  Prime will not progress. Remove it, irrigate, and recapitulate with an ISO 010 K-file to confirm patency (step 10).

2  Small to length. Passively, in the presence of sodium hypochlorite, no more than 2–3 gentle amplitudes of about 2–5 mm; irrigate and repeat (step 12).

3  Prime to length. The same gentle in-and-out motion, to optimise the shape (step 12).

Questions we get asked

Is the Small the first shaping file?
No. Step 8 is emphatic: “ALWAYS begin shaping with TruNatomy® PRIME”. The Small comes in only when Prime does not progress easily, or for severe curvatures and narrow canals.

Which points and cone after the Small?
Dentsply Sirona finish with Prime after the Small, so the Prime ones – matched to the last instrument used.

Which motor settings?
Dentsply Sirona give one setting for every TruNatomy® file: 500 rpm and 1.5 Ncm. Set it on any of the three endo motors we stock – X-Smart® Go · X-Smart® Pro+ · VDW.SILVER® RECIPROC®.

When do I throw a file away?
Dentsply Sirona’s procedure (step 11): inspect the cutting flutes routinely on removal for unwinding and straightening, and if deformation is noted, discard it and use a new TruNatomy® instrument.
